You can register in person or by mail with the attached registration form starting July 1, 2017.  Make sure form is filled in completely, the WAIVER completed/signed, and a check made payable to The Dance Workshop is attached (or completed credit card information included – a 2.5% processing service fee will be added). Phone Request & Registrations without payment will not be accepted. If there is a question or a conflict with a desired class, day or time, please call the studio office at (708) 226-5658, an alternate class will be suggested; also new classes may be added to accommodate all students on waiting lists.  An annual $25.00, non-refundable, family registration fee is required.  Only one fee is paid per family, per year.  (Copy of Birth Certificate required for all Preschool & Kindergarten registrations.) The dance season is 36-weeks. Tuition is paid in four (4), nine (9) week sessions. Tuitions listed are for 9-week sessions.  (Once you have registered and paid for Session I, you will be billed accordingly prior to the beginning of Session II, III, and IV.  Tuition for Session II, III, and IV will be due before the 1st day of class of the new session).  If more than one student is registering, please photocopy blank registration form for additional student(s) or go to our website: for additional forms.  REFUND POLICY and MISSED CLASSES: Registration Fees and Costume Fees are non-refundable.  No refunds or credits are made for absences or withdrawal from class.  Students may make up missed classes in any appropriate existing class during the same session only.  $15 Late charge on Tuition past due; $30 Service fee on all bank returned checks.  2.5% service fee on credit card refunds.
